If yes, I agree to pay $75 to Florida Station Church of God upon submission of this application. If no, I agree to clean the areas that I use. I will arrange the areas back to their original order. I will make sure the building is secured after my activity. I will check ALL outside doors after my activity to make sure they are locked.
